Nothing I attempted, including re-installing Libre Office fixed the issue. I *think* the problems started after installing either an extension in VS Code, or a LaTeX package. I had one program (Libre Office Writer) where the icon was correct, but the program name did not show if you looked at properties. The problem was restricted to my own user account. Here's what I noticed - I could create a file association and apply it successfully ONLY using ftype and assoc. It then magically cleared up while I was doing something else.
